The task is to visit the location to: 1- Verify documents and get a form filled and sign stamped by the Mfundo Arnold Lushaba Community Health Centre. 2- 3 to 4 geotagged photos are required with name board visible. 3- In case of refusal take the written refusal with stamp and get details of verifier Name, Designation and Contact. 4- If the health centre not found at given address then take a small interview of any person nearby confirming this Health centre does not exist at this address. If above mentioned requirements are fulfilled than task will be completed.
