Credentialling application

Hello My name is Tim and I live in New Zealand . I am looking to hire a web developer for an online credentialling tool. Quite simply I have here an old access database which needs to be converted into a cloud-based application. Functionality. The functionality of the access database at present is very basic! it is a simple form based interface which has reporting in the back. I will be looking for this to have form-based interface with a very sleek look to match the front end website which exists already on the cloud and it will have the ability to report from a reporting dashboard. also to include e-mail automation. The requirements are to be able to provide reporting by filtering any field in the database to include graphical reporting and or to download Microsoft Excel. Details Approximately 50 fields to form To be built in the timeframe of 2-3 weeks.
