Convert Java App into an Apple IPAD Application

I have a Java Application that I need to convert into an application that could be used on my ipad. I also need to make some small changes to the application and add the capability to upload pictures to the report and print and email the report. I have attached the application. In your description please be sure to tell me exactly how you will accomplish this task. I will only respond to direct descriptions of my project.
