A4 template for furniture specifications - repost

A4 landscape template for furniture specifications Top left 3/4 of page will be left blank for insertion of an image (in example attached is a sketch of chair showing how much room is required, this sketch is not to be added to the template). Please create a column to the right of "Image Insertion" area, the same length. In this right hand column please insert the following headings with spaces to insert text. Each heading to be on a separate line, with a line after the heading to fill in information. This right hand column needs to be 6cm wide similar to layout below. Please insert extra rows for text for"Material, Finish & Additional Notes. Specifications;__________ Length;________________ Depth;_________________ Width;_________________ Material;_______________ ______________________ ______________________ Finish;_________________ ______________________ ______________________ Additional notes;________ ______________________ ______________________ ______________________ ______________________ Please create a 5 cm Footnote running the length of the page. The bottom left hand corner will have "Item name;" with a line running 3/4 length of the page and finishing inline with the vertical margin from the right hand column begins. In this footnote in the right hand corner of the page please insert the attached Phat Sourcing logo, underneath this logo a line with "Date;" the line underneath "Version;" Attached is an example of how i would like this page setup following my instructions above.
