dental patient database - repost

i am a private dentist in Fiji with a single clinic, with 2 employess, my patient attendance is around 2000-3000 patients per annum. As a developing country, we charge minimum fees for our services, and our profits are nothing like they are in developed countries, for example for an oral exam a patient pays $5.00 or sometimes nothing. An extraction costs $20.00 VAT inclusive.

the scope of my practice is general dentistry including, oral examination, extractions, some minor surgeries, filling, basically silver and white composite fillings, removable dentures, basic orthodontics, and maybe a few other simple procedures.

the kind of software that i desire will record patient details like :

name, address, contact, date of birth, age, sex, ethinicity, medical history of known disease, allergies, vital signs on day of visit like blood pressure, blood sugar, heart rate.

i also want to include a section for patient consent for treatment where the patient can sign or input thumbprint via a signature + fingerprint pad.

for dental charting, basically a template with grids in the design of teeth layout, i would like to be able to draw whatever i require directly onto the template using a stylus. just like a pencil.

and then i need a section for notes which i can just type in, but it shoul be able to automatically record the date on which the notes are added.

after all this usually a prescription is written for medication, i will need a section for prescriptions, which will be already available in a drop down menu, where i will select the medications with dosage to go into the prescription, this drop down should also include the feature of adding new medication as required, and then there should be a print button which will print the prescrition with my company details,date, patient name, Date of birth, the selectected medications and my signature.

and then a section for printing the receipt, with charges entered manually.

the printing of the prescription and the bill sould be done with a receipt printer like the kind used in supermarkets.

i should also be able to calculate revenue recieved from the billing information.

i should be able to add new information to the patient database on new visits, include search options for records, the software should work on a windows platform, windows vista/7/8, should include support for tablets and pen inputs, networking options should be available for saving the database on a master PC, retrieving and entering information from a number of consoles within the clinic.

i have looked online and seem systems that run in the thousands. i am willing to spend USD 300-400 for a custom designed program. if this databse can work on MS Access it would really simplify things, but you would know better how to make it work best.

i may not have been so clear in explaining my requirements, if theres any queries we will sort them out through further communication. there is no major rush in getting this done, maybe within 6months. please do get back to me if you can make this happen for me at the price i deem reasonable.
