A Flash-based test

I need a Flash-based test that will include about 15 multiple choice questions (A-D), displayed one after another. After the test, a page with a summary should be presented. The test has to be quite dynamic, i.e., the questions, buttons etc. should move at their position in an interesting way so as to make the test visualy interesting. We'll send the author a few sound file to be included in various situations, i.e., after the wrong or good answer. We'll obviously provide the questions. The size of the test should be 620 X 300. I can pay for the test up to 200 USD. The task has to be done quite quickly.
