DESIGN - Logo - Header - footer - flavicon - button - banner

We need 5 elements for our new website. The website is a single page that sales a hacking tool to people who wants to catch their cheating spouse. We need: 1- a header 2- a footer 3- a flavicon 4- a "pay" button. 5- a Logo 6- a banner The header will be around a video. We have a video and it will be on top of the page, just under the header. For more info about the logo, please check the document attached to this post. The flavicon should be a smaller version (or an element) of the logo. The pay button should be in the same style as the rest, We also need a banner. It will not require more work since it will be just about re-sizing the logo. Please show us some similar work from your portfolio.
