PHP submition script for a website

Hello, freelancers, We're building a page for the users submit documents (in PDF and DOC format). The HTML of the page is built and we already have the form defined. We will have 3 forms on page: 1. The submition Form: the user will insert some informations and upload a single file (.doc or .pdf). We will need to store this information in a database, that we can access from a admin page, and also receive an e-mail notification when a submission occur. This admin page is a single-user access (no need to have user management, just a simple login and password) where we will have just one page, to list all submitions in a table, and where via a link on one of the column we can download the file for that submition. Also there should be a link to download all database content in .CSV format, and in this .CSV we should have full link to the material (so if we copy and paste this link on a browser, we can download that file). For each submition we should also receive an email (and if possible, also the attachment of submited file, but this one we can discuss). 2. Two other forms: One is a basic contact form, for the user to send us a message (a simple contact form). The other one (fixed at he bottom, right of the screen) is for the teacher to recommend the website to someone else, and this other person receives a pre-formated e-mail containing the Sender name, e-mail and message (e we also receives it as BCC). All E-mails are sent via SMTP. Language is PHP, and database is a MySQL. We will provide all HTML files and FTP/PhpMyAdmin access to you. Deadline for the project is an important issue: IT IS FOR TODAY. Our developer could not do this project, and because of it, we have this short deadline. The website needs to go live today.
