Acrobat X Pro Expert required to create fillable PDF forms

We require an Adobe Acrobat X Pro expert to create a fillable forms from a PDF document which will be supplied when the project is awarded. The document will be supplied in its native format - Word 2010 and if necessary you will recreate this in Adobe. This document has six pages that will need to be made fillable. Detailed notes describing how each field is to be used will be supplied. To bid on this project you must have Adobe Acrobat X Pro and have previous experience in creating fillable forms. With your bid please supply examples of previously completed works. The successful coder may be eligible for further work in this field as we have on-going requirements for fillable PDF forms.
