Build a Webform that writes to MS SQL

Hi, We need someone to create a web form where users read terms and conditions and tick a box for 'I agree'. It then creates a record and flags their record in the Microsoft SQL Database as yes for the field called Terms and records the date and time as well as their first name and last name. First they will sign in to our existing DNN (dot net nuke) CMS website. You will need to use the variable/parameter that DNN provides for their username as their username will then need to look up a database table of users and retrieve their employee number ID. The page will sit inside an iframe on our site. The form will be responsive, modern, PHP with Microsoft SQL Server. You will need to create a database and table to store this information as well as a table with sample user data that includes their username, name, surname, email and employee id number. Thank you,
