I need help with the following:

On the checkout page, the "State" dropdown field is missing from the Billing Information form. Because of this you cannot complete the shipping process.

The "State" field is also missing from the "estimate shipping and tax" box on the shopping cart page. I haven't touched any shipping code so I'm really confused.

I need help ASAP. Every minute counts.

Willing to pay $40 for the fix. Experienced Magento programmers only, please.
