Here is the scope of the project. ASP.NET / SQL Server The client needs a membership area - registration, login, client medical record control, and admin area. The user will be able to register on the registration page and then be prompted to pay through a merchant account (I don't have this information yet) Once logged in the user will be able to begin filling out the question and answer form which will end up their entire medical history record The form will auto save every 10 minutes Depending on the questions filled out will lead them to other questions - Can we do ajax here? All data will be stored in the clients secure membership area. If the client only fills out part of the questionnaire when they log out, the system needs to remember where they left off to continue filling out the form Once the form is complete the user will be able to print their history, save it to their computer or flash drive, or have their information mailed to them on a flash drive or cd. You will not have to worry about design at all. I will put design around it once everything is complete. FOR THIS BID I NEED: For Friday Morning(US time BONUS WILL BE GIVEN)- I will need to have finished: Registeration Page - Name, Address, City, State, Zip, Phone, Alt Phone, Email - this will go to a paypal account for now until the client gets set up with a merchant account - use info AT [login to view URL] Login page - Simple login using email and password. Password must be able to be setup by the user. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
