DISPLAY AND EDIT HOSPITAL PATIENT INFO USING ASP & ACCESS

***I could do it myself (badly) if I only had the time... *** I need a tiny ASP 3.0 / MS Access application for nurses to keep track of a dozen hospitalized patients. Includes typical user registration and password maintenance functions. Normalized tables for patients, nurses, doctors, others. Principal function is appending time and user-stamped comments to each patient's record each shift. Typical ASP features: HTML forms to search/sort/display. Drop down lists populated from existing data. Email messages generated by specific user input. Ideally would include upload dialog to store .jpg images of patient mug shots, with the path to the patient's photo stored as a field in each record. I will be using MS FrontPage to modify/maintain. Specific requirements and capabilities of 3rd-party ASP hosting site to be provided. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
