CSS and PHP Form Modifications

Hello, I need this form: [login to view URL] to look like the form in the design (image) attached, named "[login to view URL]" Then include the "[login to view URL]" page in "[login to view URL]" ([login to view URL]) * replace existing form. Then... I need this form: [login to view URL] to look like the form on the top of the page at [login to view URL] (instead of email it will say Login Name) Include the [login to view URL] code into [login to view URL] file and replace it with the existing fields. note: there is a remember password feature (I still want this - but make them link to another page to enter their email in like [login to view URL]) These forms [login to view URL] and [login to view URL] are already programmed and work fine, so please test them after making design changes to make sure everything still works. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows?
