design of smarty template for existing application

The application exist and is complete. It has been dormant for 2 years. Lately we revived the project and tried to install it locally on our wampserver, however we encountered some problems (due to the fact that we are no experts). We believe it to be a database connection problem and is a minor thing to solve if you know how. The application runs with a smarty template and we would like to have our design and layout implemented with smarty. Logo and buttons already exist as illustrator file. We are looking for a team or single person that is apt to solve both problems. the following below is the error message we encounter. As a preliminary step we would like to see the application running on a server so we can review our design to match the functionalities. Basically, the Designelements can be provided by us with Illustrator CS4. It is the implementation with Smarty that is important.
