Actionscript forms and banner

I need one banner and one variant of the same banner (the variant depending on the banner size, and in total a couple of different sizes) with actionscripting for the inclusion of forms (to capture name, mobile, e-mail, company and on the variant one: also address information) already on the banner side in order to send that to destination instead of just a simple clickTAG. E-mail address should be regexp checked against a list of know common top-level domain names so we can be more sure on whether it's a company or not. The form should also ask for input using radobuttons in the first 3 frames, and the values entered here will be calculated on a particular hidden-variable to be passed along with the form should be. I need delivery of full .fla-files so they can be changed later on if needed. ## Deliverables See the zip-file for full specs on what the frames should do, and the calculations etc.
