We have a website that we need some more work done on. The purpose of this website is for people who are experiencing symptoms that could be related to sexually transmitted diseases to go through a question and answer form to determine what they most likely have.
We will provide the symptoms to you.
What we need is something similar to stdsolve[dot]com's form on their symptom checker. The end result should be a form that is organized nicely where the symptoms are categorized into appropriate categories.
NO PICTURES WILL BE NEEDED AS YOU SEE ON THE EXAMPLE SITE WE PROVIDED ABOVE.
There will need to be one form for someone who selects their gender as male and one for someone who selects as female (they will different symptoms).
Because some of the symptoms have medical terms, we would like to impliment some Ajax into the form. Perhaps a button with a question mark on it next to the symptom. When they click on it, the content should move down and a small nice looking box made with XHTML/CSS should open with a more detailed description of that symptom.
You will be required to make the form look nice and pass the variables to a specific PHP URL using method="post" in your form. You will be responsible for implementing the form into the design we provide to you.
## Deliverables
We will be providing the design, symptoms, and anything else that you may need provided to you in order to do this task. We ask that you be willing to put in a little bit of extra time and effort into making the website look good. This is the premise behind having someone else do the work for us.
The form will need to pass along specific numerical values based on the selections of the user. I will provide these (typically a "1" if the symptom was chosen).
