Create Form, Save to DB, Generate emailed PDF

Create Credit-application We would like to re-create an finance application form. This form a) is to be accessed by viewing a specific and unique product unit (unique ID), b) contain various logic (field input value checking, if/then logic etc.), c) allow for review, confirmation, and submission, and d) upon submission create a PDF that is then emailed. Key Notes: a) Users of the form must be able to be able to hit the 'back' button on their web browser to go back in the form, previously entered data must be there! We also want them to be able to retrieve the form with their data previously provided. We want this to be as cross-platform and cross-browser as possible, so we think setting a session ID and writing to a MySQL table to repopulate the form fields is the way to go. We are open to your other technical solutions, to this requirement , and to the balance of the scope. b) Users of the form SHOULD NOT HAVE TO CREATE AN ACCOUNT until saving / submitting the form (see below) c) PDF generation should use EZPDF class.
