Creation of a custom WHMCS Report

We require a custom report for whmcs "transactions by customer type" that outputs to a CSV The report needs to prompt for start date (calendar) and an end date(calendar), and the customer type (drop down). The system will then produce a CSV with the following data for that customer type. Trans ID,Client ID,Currency,Client Name,Gateway,Date,Description,Amount In,Fees,Amount Out,Net Amount,Conversion Rate,Gateway Transaction ID,Invoice ID
