Adding Sizing Chart to Magento

I need to add a sizing chart to my magento store. I am selling customized clothing and want my customers to submit their complete size information at the time of checkout. The list of inputs that i will need from customers will be: Shoulder length, height, hips, bust, neck, waist, Sleeve Length e.t.c I won't take these inputs at the product view page when they actually buy the product because asking for so many things at this point can lead the customer to abandon the purchase. I will take this information after the purchase or between it. I need someone who is experienced with magneto programming and database. This is a small customization that i need immediately, if it turns out good there is more work to follow. please submit similar work samples.
