Hello, I'll take time and explain since you have good feedbacks and seems fair. first of all I don't see a hash code on your first page, that means if someone try and login with a bot it'll be really easy. 2) you use a GET method witch display the password in the adress tab on the second page (once we click log in) 3) I'll better use a POST method, that will check the hash in session first then check the login and password after that. 4) it'll be ajax based, so no page reload. 5) I'll suspend the login and ask for a captcha if the user makes more than 3 times a false login. I'm on freelancer only on a my free time, as my full time job I'm working at a financial company and trust me, security is our priority. I'll start today and have it done for you in less then 3 days.
