Customer and booking application for physiotherapy shop

I need a relatively basic desktop application for an injury [login to view URL] also need to be connected to a database where bookings from the website can be stored (website seperately made) this database must also contain customer information and contain a file on each customer regarding injuries and treatment which can be viewed in the program. The desktop application must contain a basic login, and four further functionality on seperate pages, including an interactive calender which appointments can be written to and an automatic email sent to the customer the day ff the appointment, a customer contact details file, and 2 treatment pages which will contain textboxes that can write to the database. I would prefer the project be written in php or java. this is a relatively small project and should be completed in the region of $250-300 and i can provide similar pictures of what i wuld like it to look like.
