We are looking for a professional PHP / MySQL developer to add more pages to our website's pre-existing form submission process. Our goal is to charge and process credit card information based on a few variables that already exist from the information submitted in the pre-existing forms. Database, tables, and fields already exist. Logic will be mapped out too. We want to create this in the shortest amount of time and swiftly. We may offer a more work, if we are impressed by your work.
Update: I forgot to mention this in the my original post, but the payment api that you will be working with to process credit card will be google checkout. Let me know if you have worked with google checkout in the past.

Shopping cart will only be applicable to Residential pickups.
After the user submits address, this is where we would like to offer a dynamic page that you will develop. Depending on the address listed, we would like to offer different prices.
I will provide database access through phpmyadmin. I user name and password will be sent to individual bidders.
One thing to be careful with when clicking through links is that a link might send you to our actual site. I went through it as much as I could to make sure all the pages were within this domain, but I could have missed something. So if something looks different it, it is most likely you were sent to our actual live website. Just keep your eye on the url.
