Create PHP Form (PRICE CALCULATOR) with Ajax from Excel File

Already have had this project posted, and it came out great but can't get a hold of person who made it to do another version. So I am posting again. I have an excel document already made that needs to be created into an online form. Custom puts in quantity and selects two options which visually updates the form (ajax) automatically, without clicking submit or going to a new page. Can show excel file if needed. A bonus is for someone who can also make it look like the attached file (and responsive to window size). *Important note, I request it to be done so competitors can not see the code behind the price calculator and rip it off.
