We need a simple form created for our facebook page to exist as a "tab" on our page. When a user clicks the tab, they get a web form where they can enter a ZIP code. When they click "Submit" the results is posted to a web page on our website. Should be very easy for someone that knows facebook. But we also want a nice looking design that is in "harmony" with our website.
