Comm-Unity Partial Care Program

Need a developer to implement a complete system for use by a mental health partial care program. A staff of 5-10 people would use this system to record all required data. It will have to run on a file server with 5-6 workstations. This system includes approx. 20-25 input screens *(for intake screening, initial service plan, comprehensive assessment, medical diagnosis, comprehensive service plan, progress notes, etc.)* Many of the fields in these input screens will require lookup screens that will allow the user to input lookup data which can be edited and deleted. There may be as many as 25-50 or more lookup screens. The system was in its initial phase of development when the project was dropped. I am looking for a competent developer to rejuvenate this project. The initial development tools included VB6 and MS access 2000. I am open to using a different development environment. I'm looking for one bid to cover all of the 4 stages. Stage 1 would allow for input, edit, delete and archiving of member information collected on forms to database and print reports on each form for hard copy to be kept in each member's file. One functionality would be the ability of the program to alert user when a form, or forms, for a particular member has to be updated. Access to a word processing program would have to implemented to print out some information or scheduling forms that can be added or modified via a word processing program. Stage 2 would entail creating auxiliary forms with appropriate lookup tables in the database in input, edit and delete data that can be selected and input into fields in the various forms. Stage 3 would add functionality to record daily attendance of members to the program billed full day or per hour. This data would be used to generate electronic billing under medicaid coding. (I have a program the does electronic billing to New Jersey Medicaid. But this program would have to produce the data that is necessary to be utilized by the electronic billing program. The means to do this will have to be discussed between you and them.) Stage 4 would add functionality to track a pre-vocational work program. it will have to account for work received from outside firms and payments received. It will also have to track piecework or hourly work done by each member. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platforms(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. ## Platform Windows XP
