Add 2 checkboxes and 3 Fields to the php-code of my wordpress theme - fast and easy job!

Dear Freelancers, I need you to do a very easy php-coding job. but please read the following description and understand it fully: USED THEME AND PLUGIN: First: have a look on [login to view URL] and check out the demo. I own this theme but already heavily customized and thats all about. Second: have a look on [login to view URL] this is the payment button which should be integrated WHAT SHOULD BE DONE: 1) look at the demo of my theme at [login to view URL] there you find the field "paypal email". there need to be added 2 checkboxes. First named "accept paypal" and second "accept creditcard/direct debit payment". the first checkbox is a mandatory field and MUST be checked. 2) when the user activates the second checkbox there need to be seen 3 more fields named "Full Name", "IBAN" and "BIC". All fields are mandatory and HAS to be filled. 3) this information need to be saved to the userprofile. 4) now have a look on [login to view URL] and click on "purchase now". in the following webpage you find the link for "Pay by paypal" and "pay by balance". there need to be a IF...THEN...routine. If the seller has chosen the checkbox for "accept creditcard/direct debit payment" then there has to be the paybutton ( [login to view URL] ) with the correct amount of money also. If the user has NOT checked the "accept creditcard/direct debit payment"-button, then the payment button is not to be seen. thats all very very easy work which is done within an hour at maximum. the named budget is fixed. higher offer will be ignored! job has to be done within a day!
