I have customers coming from an order form and landing on a thank you page. The purpose of the thank you page is to collect a list of links from them so that we can setup their accounts. The page they will be coming from will be an Infusionsoft Order form the passes through their information, email address, name, contact info etc.
The Form will have a a hidden email field that will collect their email address that is passed through. The email address will make it so that the information filled out on this form will be connected to the correct contact record.
The form will have 10 plain text fields as well, called "Listing #1" all the way through "Listing #10".
Next to each Listing Field will be a checkbox.

My web form HTML also has a checkbox for each field. The checkboxs will need to be in a new column next to each field, something like this [login to view URL] There doesn't need to be any text over or around the checkbox. I will put a header at the top of the checkbox column that explains what the checkbox is for.
This HTML has to work with the OptimizePress WordPress Theme. You can enter it into the OptimizePress Live Page Editor using an HTML element.
Also the email field needs to be hidden and collect the data that is passed through through the URL. The WordPress site already has a plugin to make the pass through feature work, the plugin is called Exec-PHP. You can use the form on this page to test passing the information through, you'll see how it appears in the URL. [login to view URL]

1. Take my attached web form code
2. Put it on my page [login to view URL]
3. Make the email field collect the passed through information
4. Format the fields to look like this [login to view URL]
5. Put a checkbox next to each field in a new column to the right of them [login to view URL]
6. Replace the submit button with an image.
This isn't a super difficult project, if you're good at this I'll likely have lots of other small or big projects for you that involve infusionsoft.
