Client needs the php form

Client needs the form in the middle of [login to view URL] to capture e-mail to a database for e-mail promotion later and also send all info to an e-mail address. This used to be in .asp before they moved it to our linux server so it needs to be done in .php [login to view URL] converting is easier asp files are available for review. Please give me an estimate of how many hours this will take to price client
