Create an Appointment maker for website.

Would like to have someone to create an Appointment maker for customers to book their pick ups on my website. I would like to add a feature so my future customers can book an appointment on line rather than call my office if they choose. . Nothing flashy just something to help my customers to use our services without having to take all the calls. General info will be needed for them to post then it gets sent to my email for me to respond. I already have something like this in another website but will want to improve the look and function. The winning bid will get the wed address to see that appt maker. Also will want to have the website I designed to be upload as well as have the tabs to function. I have "BOOK ONLINE" button on several pages of the website and want to have that working once its uploaded to my new hosting site.
