B2B Appointment Setting & Optional Telesales

We are looking for a B2B telemarketer and/or telemarketing company to set appointments (and optionally close sales). This is an initial campaign. If you are successful, there will be HUNDREDS more.
Your target market will be business owners and executives.
We can provide lists of potential targets. You are welcome to provide your own lists. You must have experience getting past gate keepers to reach the business owners and/or senior managers.
We will provide scripts, but welcome your feedback, especially if you want to close the sale and make extra $$$!
Requirements
Professional telephone manner.
No accent, American accent, or English accent.
Call forwarding (e.g. transfer warm leads to us).
Business hours 8am-4pm PST.
US providers preferred.
We are paying:
$15 per appointment; plus
$40 bonus for each appointment that leads to a sale (e.g., total $55 when an appointment results in a sale)
Per sale when you close (e.g., you close the customer over the phone and get their payment)
$100 for 1-5 sales per week
$125 for 6-10 sales per week
$150 for 11+ sales per week
Our initial budget is $600 with more available based on performance. If you are closing sales, there is no limit.
