Build a HTML prototype of a responsive UI form based on wireframe

I have a wireframe of a form. I need this wireframe to be coded into a HTML page that I can use to test with customers. My wireframe is about 80% coverage of what I need, so will expect freelancer to 'figure out' some pieces not documented.
The expected output is all the HTML and CSS etc so I can host this myself.
It needs to be;
- Interactive: The behaviour of the form, the help callouts, sliders and buttons etc needs to feel realistic - but at this stage I don't need the data to be put in a database. I will want to see the validation behaviour, ideally in-line validation.
- Responsive: I need the form to be responsive. Happy with bootstrap snap points. Please note the wireframe only covers a basic 1080px width layout, so some common sense will be needed to help in translating that to a mobile appropriate screen layout.
In terms of design, I don't need to worry about colours / branding at this stage but do need a 'premium' feel - I like this UI pack [login to view URL]
