Convert a Template to a Website

I have previously had a website for this purpose but wanted to refine it. i have attached a table that contains the fields that i would like to include, although there may be some adjustments. While I have indicated which fields should be required, i would like to be able to turn these requirements off and on on all fields. For the photograph upload, i would like the program to require that the photograph be a picture and not a word document or PDF. The website account exists; I can forward the information page when the work starts. I need for the website to send me an email containing all of the applicants info, including picture and uploads, in a form that resembles an application. I would also like a control button that will allow me to download all applicants in an excel file for review. This website is purely for people to apply for a job, so it does not need to be discoverable on the internet. The user will be directed to it from an advertisement. PLEASE ASK QUESTIONS BEFORE BIDDING AND ACCEPTING THIS ASSIGNMENT. I would like turn around in a day or two.
