Campaign Monitor Email Template - Design to HTML

I have here a simple email campaign design that needs to be converted to a working template for Campaign Monitor. Winner will need to provide some examples of other email templates done for Campaign Monitor. Template must be coded to work in as many email clients as possible. Priority being Outlook. We hope to have this turned around ASAP. It should not take more than 2 hours of work to complete. Image of the template is attached.
