Hi, I have had many years experience in electronics design, especially with microcontrollers. Most of my current project work involves at least one Arduino compatible AVR. I have extensive experience in PCB lay out and can design and create the documentation you need for a third party fab to produce your boards. The ADT7470 is an interesting chip. A brief look at the data sheet shows its easy enough to provide what you have asked for directly. The biggest question would be what kind of power supply you intended to use to power the fans. The micro parts might just as well be a module type like the 'Nano' or one of the Teensy devices, easy to get and easy to use. As with most projects of this kind you need to be thinking of how you want to manage the physical plugs and connectors to get signals and power on and off the board. (big factor on size).
