SEO friendly mobile version of homepage

I would like to have a mobile friendly version of the following site: [login to view URL]
I plan to continue to use the existing site for none mobile access, because it is doing very well regarding SEO. But for mobile users I would like to redirect them to a new site that is mobile friendly.
I have the following requirements to the new mobile site:
1) It should pass the Google mobility test: [login to view URL]
2) It should contain the same information/text/images/logos as the existing site.
3) The existing SEO is very important, and therefore most of metatags, internal links, headings etc. must survive.

5) The most important SEO key words are: "tandlæge", "tandlæge aalborg" and "aalborg tandlæge" (tandlæge means dentist, and Aalborg is the city)
6) It is important that user can press phone number and email in order to contact us.
7) The only new feature is that i would like a new contact form with the following items : name, address, email, mobile phone, CPR number (text field) and message. It should post to a dummy php scripts saying "Not implemented yet".
8) Information should be provided how the existing site should be changed, in order to redirect traffic from mobile phones to the new mobile friendly site.
9) I expect the new site should run https, and perhaps the existing site will also be changed to this.
10) Code and comments should be written in English. English screen text is ok in cases where you are in doubt, i will translate it after delivery.
11) The new mobile version should be self-contained in a subfolder below the root. In the future I might move the content to the root folder, and delete the existing site. Therefore it is also important the the site looks ok on a PC screen.
