Need a webapp for self-assessment quiz

I need a self-assessment model for DevOps which I can host locally on my machine. You may use any tech stack, be it asp.net, MS SQL, Angular or whatever you prefer.
The idea of this assessment would be based on this picture:

App structure:
1. There will be sections like mentioned in the above picture i.e. Culture, Design & Architecture, Build and deploy etc.
2. There will be maturity levels like mentioned in the above picture i.e. Base, Beginner, Intermediate etc.
3. Each section will have different questions that will decide the maturity level. If you see the picture, you will understand how that matrix works.
4. There should be a backend (UI or config file) to add, update questions.
5. Each question should have answers respective to its maturity level so if user chooses option (a) as answer, it should be calculated pertaining to the response shown in the grid.
6. At the end of this quiz, user should be presented with a summary of questions and answers given by user, and an option to download the response report as PDF. Use Chrome PDF Printer for this need, unless you have something else.
7. User Interface should be simple and customizable with a CSS.
8. Provision for a logo and title on the webpage.
9. At the start of the quiz, user should be able to put Team name, Project Name and Team Lead name along with date time, and it should be saved in the database.
10. It should be possible to see a result page in admin panel where all the answers given by a team shall be fetched from the database.
11. Remember, you can just create a prototype of this application, you dont have to write all questions and answers. Once an interface is available, I can do it myself. All these questions shall be saved in database of course, and new questions shall be added whenever needed, with edition of existing ones.
