We require a form to be incorporated on an existing Opencart website for certain products. The form has to be able to be incorporated in the admin area so the client can just tick the box if the form is required for that product. The form will be stored on the site for each client that fills it in so they only need to fill it in once. The example of how it works can be found on their old website ([url removed, login to view] ) - You will need to create an account to see it. Please note this is their old website not the new Opencart site. There is only 1 questionnaire to be created that will be applied to multiple products as required.
