Online HD Video calls, meetings and appointments for health professionals

The successful applicant should have HP / MySQL / AJAX / CakePHP / Laravel / OOP / MVC / Developer skills and I believe the current site is in Cake PHP I want to create online consultations between Doctors and patients and we need a robust video streaming system and I am leaning towards twilio The site works like this and there are 2 groups 1) Doctors will join and create a profile and you will find a rough draft here [login to view URL] The doctor will add their information and pricing as well as all contact details including mobile phone number and PayPal address. Doctors will set their own schedules or can simply login in when they want to accept online video consultations. When a patient has booked an appointment they are taken to a virtual waiting room and the doctor receives both an email and SMS notifying that a patient is waiting for them in their virtual waiting room. The email and SMS has a link so the doctor can simply click and they are then taken to the online consultation So much like an online pizza store, the doctor creates a profile, sets their price and when available this will be displayed in the main search area. 2) Patients will enter a post code or city and a list of the closest doctors will appear again much like an online takeaway or home delivery site. The patient reads the reviews from previous patients and then decides on a doctor. They request either an immediate or book for another time. They are taken to PayPal and pay for the consultation and then if immediate taken to a virtual waiting room. The doctor is then notified by SMS and email that the patient is waiting and when he or she clicks on the link in the email and sms the consult begins. The patient is also sent the same link via email so they can invite a family member or anyone they please to join the live consultation After the consultation the patient can review that doctor. If they do not have to review at end of consult they can do so later via their member area if they have registered. If the patient requests a booking for a later time or day they also receive a SMS and email notification 15 minutes before the consultation begins. Once they are in the virtual waiting room the doctor then receives the same SMS and email notification. If the patient wishes for someone else to be present they simply forward the email to that person and we can have a 3-way consultation So to summarise think of an online takeaway site where people enter their post code or city and then a selection of stores are listed. The person can enter Pizza, Indian, fish and chips etc and those shops selling those types of takeaway dishes are displayed and the same with doctors and specialists. The person clicks on the shop/doctor and then they read the profile and reviews from others who have used before. If they are happy they purchase their food but in this case an online video consultation. Let me know your ideas and how you would approach this. Please also show me any links to relevant work
