DID Pool Handling with Changing Routes

I need custom programming in PHP that can accomplish the process mapped in the attached process map. The goal is that someone can complete a web form and when it is submitted and validated, a DID is provided to the person completing the form and when the DID is called, the call routes to the right IVR (based on the data from the web form). All actions must be logged to a database.
