**Summary:** Using a sharp-looking design of your choice, create html forms to match the basic look and layout of forms in a given MS Access database.
**Description of Work:**
Create a simple design scheme that is somewhat similar to the look-and-feel of the forms in the given MS Access database (use similar colors, type style, etc., with fairly wide flexibility for the sake of any design improvements, greater ease of design, and compatibility with the Web format). Using that design scheme, create html forms to match the look and layout of each of the forms in the database. There are 45 forms of varying complexity, some with 5 lines of text and a few with over 20 fields and labels.

Note that this project does not involve porting VB code or the like; just the form layout itself.

**Ideal Candidate:**
The ideal candidate for this job has the following characteristics:
1. A good sense of artistic design and layout;
2. Experience with page design for the Web;
3. A high level of proficiency with style sheets (CSS2);
4. Familiarity with Microsoft Access form structure and properties.
**Important:** Bidders should submit at least one sample of similar pages you have created before, including style sheet.
## Deliverables
1) One html page for each form in the given database, plus the style sheet used therein. 2) Complete ownership and distribution copyrights to all work purchased.
