Database + form in webpage

We need a nice and good looking form to embed in a website: [login to view URL] ; specifically in this URL: [login to view URL] Then, we need that this info can be accesible in a private url, with an user/password. This results shown as a list, and with a button with wich you can export all the list to excel. This list should have this options, for each record it should be possible to: export record, modify record and delete record. (To add a new record, through the web). The form is in spanish. There are different types of fields. The form should have the look and feel that I propose, not the one is now. ## Deliverables Here is what I did: [login to view URL] It works more or less right, but what I really need is to look more o less like the ZIP file, but organized well and most similar to this document.
