Build lead generation form for Wordpress site

I require someone to code functionality and design for a multipage lead generation form for my Wordpress site. The form will then need to be installed within my Wordpress theme. (Please note: Once a user completes the form it can simple be sent to me via email.) I also have the .psd file formats for the form. The form should work exactly like the form located here: [login to view URL] Please review this form [login to view URL] and pay particular attention to: > the animation between screen changes, > the way the images change when selected > the way the continue button pops up from the bottom of the screen The form will also have areas with drop down menus and certain points where emails and phone numbers need to be varied. Integration with either mail chimp or campaign monitor is also required so that sign up confirmation emails can be automatically sent.
