Static HTML Page + 2 forms (based on similar page)

I want to use an existing site as a template and change it for my needs. The site is just a single page and needs only to be changed very slightly. I have done a very basic mockup of the changes. In addition, two forms need to be added that basically create emails and update a simple database. ## Deliverables I have attached a mockup of the changes. You'll see the form at the bottom. The form must email the information and also store the data in a database table (1 single table mysql) The images and text are just place holders. The major changes are: 1) Replacing current video embeding with YouTube video without changing the way it looks 2) Changing the placement of some of the image and text objects 3) Creating the form at the bottom of the mockup you see below and having that form send a wellformatted email to the admin and also store form information in database. 4) The live chat feature is something we already have a code for so don't worry about that 5) Another simple page containing only 1 form is needed e that basically only gets two fields from user: Name and Email and stores them in a database table and emails admin. We're going to then use this page in an iframe within facebook to collect emails. The page must then redirect to another page. ***NOTE:It is not very necessary to worry about the text because I can edit that myself. Just give me the required space to do that.
