Need to make 2 wordpress compatible PHP Form

Hello, Here i attached 2 screenshot with my job description. Please have a look that first then read the job description bellow. We need 2 different form by using PHP, Javascript/jQuery. You don't need to create any admin panel/area for drop-down area which i mentioned in screenshot. You will just create 2 form and functionally active. I think you should not use mySql too.. just scrap the data from a text file. It would be better to use in wordpress. Or if you can access wordpress db then it would be great too. For [login to view URL] there need a Checkout page with 2 payement method which i mentioned in image. For [login to view URL] , you can create a text file or xls file to save zipcode and when it does not match then show a custom text as like screenshot.
