Clone my instapage landing page

I have created a decent landing page for the purpose of lead generation at bisnesideal.instapage. com. However I cannot afford premium instapage account to suit my autoresponder. Therefore, I need someone to religiously convert the single page to HTML/PHP webpage and integrate my autoresponder code to the e-mail form in the page. Please see the attachment for the code. I wanted him/her to design like the same and preserve all the fonts and wordings as original site while improving the site loading time. This should be an easy task even for a beginner. Bidders who are in keen interest to solve this should come up with a demo website just like mine. The best will be selected base on the design and functionality of the website and of course with acceptable budget in the shortest time. I am expecting a budget of less than $40 for this simple project. The outcome will be the file package that I will upload myself to my website, bisnesideal .com.
