Convert PDF agreement in online fill in project

Looking for a script / code that can make our contract/agreement filled in online based on the given input with calculations or not and when click on send a PDF will be send to client and me in a PDF (with our design) with a small text in the email It will be 2 contracts so basically 2 scripts based on a menu choose what contract they want All info must be kept also in a database and with a click be downloaded
