Create Custom forms in Woocommerce (mapping required) and extract data

Task: CREATE A 3 PAGE WORKFLOW WITH CUSTOM FORMS - AND EXPOSE DATA CAPTURED. - STEP 1: Create 2 custom forms in Woocommerce (both have same structure) where customer will enter data as the first step of the order capture process. the product description is not visible, so there will be some mapping of the form fields in the custom form to a product. the mapping is not complicated. STEP 2: setup customer login page. (simple task - standard WP login, or social login) Data Storage and access: - must e able to link the data entered to the customer and record the data entry date -the data must be store by customer by product, and must be accessible either as a woocomemrce backend table or by using CSV or excel export in the format attached. Attachements: Mockup of workflow and custom tables (task is STEP 1 and 2 only) Product list (all products are variable subscription products) Exmple of field export with order data captured.
