Fix and add feature to existing PHP application

We have a 3 page PHP application which was created using an application builder. The builder is not perfect and requires some adjustments. The first page allows you to select a template and includes 3 filters. This page is functioning. The second page need to display the template name, the application associated to that template and allow the user to change the user id, update the allow deletion flag and update the template locked flag.
The delete key, which already exists needs to delete the template BUT before deleting need to make sure it’s not in use by any schedule job canned reports. We have similar functionality in another application (see table gbs_canned_reports_scheduled_jobs). The third page already created contains the code for the table update/insert.
Currently the selected template from the first page is passed to the second page but the form is picking up the first record.
Working on this project will require you to connect to my desktop and perform the work. This MUST be completed during the hours of 8:00 AM – 3:30 PM EST.
