I need to fix mailchimp plugin on our site. We are using a theme and put in mailchimp api key but the field on the frontend where user is supposed to type email address doesn't show up. Also we need to add a page with a simple form within the same template of our site, which after being filled, will email results to designated email address, and will go to next page which has a table of information provided by us.
